A patient portal is a safe and secure way for patients to communicate via web / e-mail with our practice. You can electronically communicate with our office, view test results and update personal information.
This service is provided for our established patients only. If you are already an established patient and have not received information on how to access the Patient Portal, please ask for more information at your next appointment or contact our office directly at (951) 698-1901, option 1.
If you are a new patient to our practice, please complete the New Patient Forms in advance of your appointment. Once filled out, please either send the completed paperwork to [email protected], fax to (951) 698-1074 or bring into our Murrieta location two days prior to your appointment.
